This book is a collection of papers which innovate the theory, method and practice of the safety, reliability
and risk management of technological and engineered systems. Innovation is all about bringing change and
improvements, often to something established. All papers have been contributed to the 26th European Safety and
Reliability (ESREL) conference, an established meeting of scientists, academics and practitioners held annually
under the auspices of the European Safety and Reliability Association (ESRA).The papers contributed to ESREL
2016 continue its tradition of sharing new ideas, methods and applications across multiple industry domains.
The form of innovation varies across topics with relatively incremental change associated with those better
understood problems and more radical innovations proposed for emergent problems or modelling challenges
facing industry and society. This book contains papers on long-standing topics including risk management,
reliability modelling, maintenance optimisation, probabilistic risk assessment, human factors, and more, as
well as papers on contemporary themes, such as cyber-security and systems resilience. Practitioner authored
papers provide insights into the challenges faced in real applications and share insights into the impact of the
interventions to manage risk, while scientific papers share theoretical, methodological and empirical research
contributions.
