It is anticipated that advances in information technology (IT) will contribute to improving the quality and continuity of the health service, and enhance access to information. Nurses have an obligation to ensure continuity of patient care when patients are transferred from one health care delivery level to another, for example from hospital to home. This study looks at what information must be exchanged in order to achieve and ensure continuity of patient care. The results of an ongoing prospective study, focusing on the content of the nursing discharge note prepared for electronic implementation, show that nurses mostly record and exchange medical and clinical information, with less emphasis on psychosocial and continuing data.
